Parishioner request for Certificate of Eligibility Form
A Certificate (sometimes called a Letter) of Eligibility is necessary for each Catholic Godparent or Confirmation Sponsor. This certificate is to be obtained from the parish at which the godparent or sponsor is registered and presently worshipping. It must be the original certificate stamped with the parish seal and signed by the pastor. This certificate indicates that the godparent or sponsor meets all the requirements, including attending Mass regularly and having received all of the Sacraments of Initiation (Baptism, Eucharist, and Confirmation).

The person who seeks to be a sponsor or godparent must complete the following form to submit your request to St. Ann Parish. Please allow 3-4 weeks for processing.

Please note that the form below only applies to registered parishioners of St. Ann Parish.  IF YOU ATTEND A DIFFERENT PARISH, YOU MUST CONTACT YOUR OWN PARISH FOR THE NECESSARY PAPERWORK.

Solicitud de feligreses para el formulario de Certificado de Elegibilidad Es necesario un Certificado (a veces llamado Carta) de Elegibilidad para cada Padrino Católico o Patrocinador de Confirmación. Este certificado debe obtenerse de la parroquia en la que el padrino o patrocinador está registrado y rinde culto actualmente. Debe ser el certificado original sellado con el sello parroquial y firmado por el párroco. Este certificado indica que el padrino o padrino cumple con todos los requisitos, incluido asistir a Misa con regularidad y haber recibido todos los Sacramentos de Iniciación (Bautismo, Eucaristía y Confirmación).
